You may either fill out the forms on your computer
or print out the forms and fill them out by hand. To
fill out forms on your computer, download the Microsoft Word
Version. Press on the first gray text box and fill in the
information. You may move to the next gray box by using the
"Tab" Key. Save the file on your computer, and email
